Health Assessment Award Increased
In 2008 the UC Office of the President launched the new StayWell benefits program as part of the expanded UC Living Well program. Under this program, eligible UCB employees and their spouses enrolled in Blue Cross, CIGNA, Core, and Health Net health plans can receive a $100 gift certificate for taking the StayWell online health assessment. Additionally, if you redeem the entire $100 gift certificate with Rec Sports, you’ll receive $70 off the cost of a Rec Sports membership. Current members will get a free two month extension. Offer is good through April 15, 2009.

CUE Members Now Eligible Too
The University and the Clerical and Allied Services Unit represented by CUE (“CX”) have agreed that CUE’s members will have access to StayWell effective March 2, 2009. CUE members will be eligible for the incentive March 2 – June 10, 2009.

- Within a week, you will receive an email (or letter if you did not provide an email) from Hallmark Insight with a link to a website where you can choose your $100 gift certificate.
Select UC Berkeley Recreational Sports from the Sports Category. Your gift certificate will arrive in 3-4 weeks.
- Bring your gift certificate to the RSF cashier where you will receive $100 in Rec Sports services, PLUS $70 off the cost of an annual Rec Sports membership.

You can use your $100 gift certificate toward most Rec Sports services*. For example, apply it toward a Cal Massage Session; Personal Training introductory package; Windsurfing Lessons; Instructional fitness classes, such as Pilates, or camps for your kids. To redeem your incentive reward, come to the RSF Cashier. Note: to purchase camp sessions, please come to the Strawberry Canyon Enrollment Center. You cannot apply the $100 gift certificate toward a Rec Sports Membership.
You'll also receive $70 off the cost of an annual Rec Sports membership. Current members will get a free two month extension. Please note that to receive $70 off an annual membership, you must redeem the entire $100 gift certificate with Cal Rec Sports.

- Take the health assessment.
- You’ll be automatically entered in a quarterly drawing with a first prize of a $500 gift certificate to spafinder.com or a sporting goods store and six chances to win a 80GB iPod®.
- You will immediately receive an email from KaiserPermanente@HealthMedia.com confirming completion of the HA.
- Print out your confirmation email and bring it to the RSF cashier, along with a valid form of ID, where you will receive $70 off the cost of an annual Rec Sports membership.

About Your Privacy
You will have the option to decide whether or not to share your personal information with your Health Plan. You can exercise this option by indicating your preference at the end of the assessment.
The University of California (“UC”) will never receive personally identifiable information about you from StayWell or from Kaiser. UC will receive only de-identified information about the program as a whole. That de-identified or summary information will be used by UC for the purposes of reporting, research, and/or wellness program planning.
